This is subject to mass change, as I have no experience in actual jobs or people tell me ways to make this at least a little less overcomplicated.
This is overcomplicated for a reason, pointing out every little detail helps to avoid confusion. There is a tl;dr at the bottom of the screen for people who need a less complicated mine.
Hello, I'm _Ivy and I'm renting myself for Excavation.
Please put your Username, co-ordinates, nearest region post and name, and time you want me to be there in the spreadsheet.
I'll be expecting the following:
-I'll be expecting to be provided with a pickaxe, please inform me if you want me to bring my own. Bringing my own pickaxe is 500 rhines and I can't do silk touch since I have no books.
-I'll be expecting to have an rough layout of where I need to excavate, we can converse at the location for further details.
-I'll be expecting to be paid fairly, although there are multiple ways to pay me.
--My default prices are 1 block = 5 rhines. Not including things like pickaxe.
--My alternate prices are a debatable "Lump sum" where we agree on an estimated amount with estimated blocks. Others may find this method of payment more reliable because they know what price it is and have a chance to save/lose money, this price is open for debate though so unless I see it reasonable you shouldn't expect me not to level the price up.
-I'll be expecting that if you want me to repair the pickaxe (eg it has efficiency and silk touch) you tell me.
-I'll be expecting a storage system and whether you want me to chuck cobblestone, specific ores etc. This can be a simple double chest array if you want.
I have the following policies.
-You must give me permissions to mine, place beds etc
-Any and all ores that I find belong to you. You will be given these without confusion.
-I have permission to postpone times, although compensation will be applied similar to the insurance.
-My rent out times are between 5:00 to 9:50 UST, my limit is 1 and a half hours so 2 hours would be +500 rhines, 2 and a half would be +1k etc
tl;dr - Just put the stuff on the spreadsheet and give me a pickaxe or two to use. 1 block = 5 rhines and you can also tell me what you want to pay for it and we can settle on a price.

I think this is a nice idea. Hope you get some clients this way.
